Quality improvement Specialist

Department: Office of Quality and Patient Safety
Location: Far Rockaway, NY

Pay: $115,000 - $125,000

Job Summary:

The Quality Improvement Specialist will play a pivotal role in driving quality initiatives and enhancing patient safety at Episcopal Health Services. As part of the Office of Quality & Patient Safety, this position requires a highly motivated individual with a strong understanding of quality management principles and a passion for improving patient care and reducing harm.

Responsibilities:

  • Coordinate and implement the organization's Quality Management, Patient Safety and Performance Improvement Programs.
  • Recommend organizational objectives to enhance quality and patient safety, aligning with strategic priorities.
  • Assess the quality of care in outpatient and inpatient settings and collaborate with multidisciplinary healthcare teams to enhance care delivery.
    • Develop and implement quality improvement plans and strategies
    • Conduct root cause analyses to identify and address quality issues
    • Monitor and analyze quality metrics to track performance and identify areas for improvement
    • Develop and implement quality control procedures and standards
    • Facilitate quality improvement teams and workshops
    • Collaborate with cross-functional teams to drive continuous process improvement
  • Participate in projects related to quality and patient safety, assisting in recommendations and coordinating implementation of corrective actions plans.
  • Conduct clinical quality of care reviews, health outcomes reviews to ensure if standard of care was met.
  • Prepare variance reports for leadership, physicians, and other team members as needed and present in interdisciplinary committees.
  • Identify workflow barriers, patient-specific issues and concerns to appropriate leadership and improvement teams and department leaders and cross-functional teams for resolution.
  • Provide expertise in TJC & CMS core measures, eCQMs, GWTG and indicators monitored by regulatory agencies, publicly reported quality reimbursement programs
  • Monitors performances of all publicly reported and internal performance data to identify opportunities for improvement
  • Conduct gap analysis on existing policies and practices and research best practices to make necessary recommendations
  • Conduct focus studies to measure and assess patient care services' process stability and workflows for achievement of patient outcomes.
  • Conduct education on quality management and performance improvement theoretical frameworks.
  • Assist in developing appropriate measurements to assess processes and patient care outcomes.
  • Collaborate with clinical and administrative leadership staff to ensure compliance with regulatory and professional standards.
  • Review and evaluate the Performance Improvement Program and the Organizational Performance Improvement/Safety Plan as needed.

Qualifications:

  • BSN nursing degree required. MSN Preferred
  • Current New York State Registered Professional Nurse, Nurse Practitioner, or Physician Assistant license.
  • Three to five years of clinical experience, including general knowledge of Quality Management principles, NYSDOH, CMS, and TJC Regulatory reporting requirements
  • CPHQ and CPPS certifications preferred. If not, certification is required within six months of employment
